We collect personal data directly from you via registration forms, online applications, telephone calls, and correspondence. This includes information such as:
- Full name
- Date of birth
- Address and contact details
- Direct Debit mandate instructions
For plans that include access to the Internation Dental Emergency Scheme World, we may also collect sensitive medical information from you or your dental practice, subject to your explicit consent where required.
Information from Third Parties
We may obtain personal data from reputable third-party sources and public records, provided it is legally permissible and GDPR-compliant. Where consent is not explicitly provided, we conduct a legitimate interests assessment to ensure lawful use.
Use of Personal Data
Your personal data is used to:
- Administer and manage your dental plan
- Process Direct Debit payments
- Handle requests for assistance under the Scheme (if applicable)
The legal basis for processing your data is the performance of a contract. We are committed to protecting your data in accordance with GDPR and the professional obligations of your dental provider.
We do not use your personal data for marketing purposes.

International Data Transfers
We do not currently transfer personal data outside the European Economic Area (EEA). Should this change, we will ensure that appropriate safeguards are in place.

Personal data is retained in accordance with the Wesleyan Assurance Society’s retention policy. Generally, we retain data for up to 12 years following the end of our relationship with you, to fulfil contractual, legal, and regulatory obligations. Data is regularly reviewed and securely disposed of when no longer required.

Complaints Handling Procedure
At Plan4Health, we are committed to providing the highest standards of service. If you are dissatisfied with any aspect of our service, we welcome the opportunity to resolve your concerns efficiently, fairly, and transparently.
1. Acknowledgement
We will acknowledge receipt of your complaint within five working days, confirming that it has been logged and providing you with a reference number for future correspondence.
2. Investigation
Your complaint will be assigned to an appropriate member of our team for a thorough and impartial investigation. We are committed to handling all complaints with objectivity and care.
3. Response
We aim to provide a full response within ten working days. If the matter requires more time to investigate, we will keep you informed of our progress and provide an updated timeframe for resolution.
4. Outcome and Resolution
Once our investigation is complete, we will contact you to explain the findings and any actions we intend to take to resolve the matter. If you are satisfied with the outcome, the complaint will be formally closed.
